Introduction Polymyalgia rheumatica (PMR) is an inflammatory disorder primarily affecting the shoulders and hips, leading to pain, extreme stiffness, functional limitations, and potential complications such as giant cell arteries and respiratory issues. Due to symptom overlap with other musculoskeletal conditions, PMR is frequently misdiagnosed, delaying appropriate treatment. There is currently no definitive test, but diagnosis is typically confirmed by a rapid response to prednisone and elevated inflammatory markers. While pharmacological treatment with glucocorticoids is well-documented, there is limited evidence supporting the role of physical therapy (PT) in managing PMR. Purpose The purpose of this case report is to explore the successful treatment of a PMR patient using physical therapy. Methods This case describes a 69-year-old male diagnosed with PMR after experiencing progressive bilateral hip, low back, and right shoulder pain following a fall. Imaging revealed degenerative changes, and the patient was prescribed PT alongside prednisone. Interventions focused on progressive strengthening, manual therapy, and functional training, targeting deficits in mobility and strength. Results The patient’s Modified Oswestry Index (MODI) score improved from 28% to 8%, demonstrating reduced disability and increased activity tolerance. The patient reported improved ability to perform daily activities, decreased pain, and enhanced overall function. Discussion This case highlights the potential benefits of PT in managing PMR symptoms when used alongside pharmacological treatment. Further research is needed to evaluate PT as a standalone intervention as it is currently very limited. Additionally, increasing awareness of PMR is crucial, as its high misdiagnosis rate often leads to delays in effective treatment
