Description

Endovascular revascularization (ER) is a common vascular surgical procedure used to provide treatment for lower extremity peripheral artery disease (LE-PAD). This procedure is done thousands of times a year on Louisiana Medicaid participants, many of whom suffer from a type of LE-PAD called intermittent claudication. Numerous national and international guidelines call into question both the utility and cost-effectiveness of endovascular revascularization as a first-line treatment when compared to conservative therapy. Appropriate medication, exercise therapy, and lifestyle modifications have proven to be effective in providing long-term symptomatic relief and to limit progression of the disease. The policy outlined here sets in place guidelines for appropriate use of ER in the setting of intermittent claudication to ensure that patients adequately undergo conservative management prior to receiving surgery.
